Option[@"Save.File"] Not setting file name

Most of the time, this works fine. However, I'm running into an issue on certain PCs where I set the Option[@"Save.File"] and it's not getting retained or used when the file actually writes to disk. So when the save event happens, the file name in ".\Untitled.pdf". It's correct in both the OnEndDoc and OnDocSpooled events.
What should I be looking at to figure out why this isn't getting set?
