Many Users receive attachments like pdf, word, excel and so on as email in Outlook.
Is there any possibility to add a custom PDF tool to outlook ribbon pdf-xchange to start a pdf tool process?
For our example, we want to export a pdf in word.
interesting is, that i can drag'n'drop a pdf from outlook to a pdf tool shortcut and starts the conversion to word document

No - there's no integration of PDF Tools into outlook.
I am not aware whether there are any tools inside Outlook itself that could e.g. make you execute a specific command - if that is possible (this would be outside of the scope of the help we can provide) - then maybe you can add such custom buttons and have them launch a command line for the specific tool, with the PDF file attachments as parameters. Here are the command line options for starting a custom PDF Tool:

is there a way to configure the pdf-xchange printer so that pdftools.exe can be called with the appropriate parameters when printing?

Hello tomt,
You can create separate instances of the printer driver - each with it's separate settings, and then just call a different "printer" name from the list of existing ones:
