Hi,
I am not sure if this option is available, so I'll write the idea:
During the phase of document (e.g. agreement) preparation in Word it is possible to generate Tabel of contents together with PDF, e.g. based on styles etc. It is quite often that later on document is printed out, signed and scanned back to PDF!
The idea is to export Table of content (bookmarks) from the first one to scanned one.
What do you think about it?

This should be possible with PDF-XChange Editor.
In the PDF generated by Microsoft Word, by using the "Export All Bookmarks ..."-entry in the menu, after clicking the "Options"-button (wheels icon) in the Bookmarks pane (View > Bookmarks). Creation of a *.xcbkm file.
In the PDF with the scanned document, by using the "Import Bookmarks..."-entry, and importing the *.xcbkm file.

I do this quite a lot.
I believe the easiest way is to simply select all bookmarks in the source document, copy them to the clipboard, and paste them into the (empty) bookmark pane of the target document.
