How to create form from PDF document

Is there a way to automatically create a form from a PDF document? I'd like to avoid placing and formating all the boxes manually.

Any suggestions about shortcuts to formatting new text boxes and check boxes?

Thanks for the post - I'm afraid that this isn't possible at the moment, but is something that we plan to implement as I understand. It's quite difficult to implement though, so I cannot give any time frame at this point.
