Form fields not highlighting correctly

Hello. I'm trying to fill in a form (attached). When I click a box under the Identification section (e.g. "Individual") only the First Name field on that row becomes available. In Acrobat the behaviour is as expected, i.e. all of the fields on the row become available.

Could somebody at Tracker take a look?

Thanks for the post - I see the same issue here and have created an internal development ticket to track the progress of the issue (RT-4701). The good news is that the row can still be filled out, it just doesn't highlight correctly and all rows can always be filled, instead of being disabled as they should.
