Forms: How to Convert Excel to PDF Form
Is there a way to convert an Excel document to a PDF Form? My document has a couple hundred check boxes, will they be converted also?

You can open your Excel file with PDF-XChange Editor.
1) Everything what is in the Excel cells will be converted to "text".
2) Everything what is Form or ActiveX controls will be converted to "images".
Myself, I am afraid that - with the current version of PDF-XChange Editor - you will have to manually re-introduce all the needed check boxes at their required positions.

Yes, You will need to reintroduce these Checkboxes manually, as willy said, they may convert as images, but they will not function as toggle able objects without that.
