The bookmarks item on the menu does not have a shortcut underlined when the Alt key is pressed.
The Custom Scan command and the Insert Scanned Pages command use the same icon.

The Bookmarks menu is loaded from a plug-in so that's probably why it behaves slightly differently, I will ask our devs to check.
As for the same Icon - it represents a scanner (flatbed one) and a small + sign to indicate that you will add from that device, so I believe having the same icon there is fine, as the operations performed are similar as well.

The Bookmakrs entry in the main menu will have an underscore under the 'B' from next build onwards. This was something we had to fix, and a colleague from the dev team just sorted it out!

There already is an icon that does not have the plus sign that could be used. It is utilized as a part of the "combo scan icon" pictured below. To me, it would be more logical to use that icon for Custom Scan. That would differentiate it and the Insert Scanned Pages icon when using them side by side on a toolbar.

That's a reasonable request as the two commands call the same dialog. Updated the icon to the blank scanner without +.
