I have tried opening and submitting a PDF Form using PDF-Xchange.
However some of the Tick Boxes do not tick correctly - instead of showing a tick the box shows a tiny icon (which looks like multi-pages.)
These little icons are printed when I print the form also.
The form works fine in Adobe Reader , why not in PDF-Xchange.

Zipped pdf file attached. There are notes added to the pdf explaining the problems. In addition the submit button does not work and the form is not submitted.
This pdf works correctly in Adobe Reader, incuding the submit button.

First about the icon: PDF-Viewer shows this icon because such behaviour specified into the PDF file (this icon represents symbol 4 into the font Windings-Regular which specified for these check boxes). Adobe Acrobat ignores the font specified for the check-box and uses its own font (which, to be honest, doesn't conforms to PDF specification).
So, solution is here -- duplicate Adobe's bugs/features or make our viewer compatible with specification ?
About submit action: I cannot find where the trouble is while there are no submit button

You say " Adobe Acrobat ignores the font specified for the check-box and uses its own font (which, to be honest, doesn't conforms to PDF specification).

If Adobe is ignoring the specified font, then it must be that PDF-XChange is doing the same because the tick shows in one box but not the other. Therefore PDF-XChange has the same "bug" as Adobe, but only on SOME tick boxes. How crazy is that ?? The tick should show in either box, not that meaningless wingding thing.
As I said, the form submit button was removed to avoid spurious submissions to an ongoing survey, but I will try and make a different form to demonstrate that PDF-XChange does not handle the submit process.

Not exactly. In your file, for the top "Yes" check box the font "ZapfDingbats" is specified (this font is used as a rule for such tasks) and there is no problem with this check box.
The same font is used for bottom "No" check box.
BUT, for the two other check boxes, the font Wingdings-Regular is specified (and this font is embedded within the PDF).
Using charmap program (press Start -> Run... enter charmap.exe and press Run) you can find which symbols correspond to char 4 specified as a mark symbol for these checkboxes.

Attached is the zipped pdf Form with the Submit Button.
Using PDF_XChange after filling in the form and clicking Submit - nothing happens. The form is not submitted.
Using Adobe Reader after filling in the form and clicking Submit you are asked if you wish to Block/Allow access to the server to send the file. Clicking "Allow" sends the form and Adobe opens a new page with the message "Thank you, your form has been submitted"
The Adobe sent form arrives in my inbox quite quickly, but nothing arrives from the PDFX-Change form.
It would be nice if PDFX-Change could do the job too !!
Until this is sorted there is not much point in trying to solve theTick/Wingding thingy.
