I think, that's a Ditto problem, because it's the same in the "Save as..." dialogs of MS Word and Notepad2.
On pasting Ditto highlights the existing filename and then pastes, so it effectively overwrites.
You are right, it's annoying...

Maybe I misunderstand the question, but it's anyway there:
The "Save as" preference also covers "Save as optimized" and "optimized" is added to the original file name.

I appended 8 files one by one (by drag and drop from Explorer) and instead of after the last page as indicated by the red line every appended file was inserted between the last page and the page before that. Not on top like in Roei's case. Lot of moving in a 750 page file. I hope this is related and...

Confirmed - no corruption anymore with 6.0.318.1.
For me the problem was not the right click "Open With" menu being empty, but almost everything else in the the right click context menu was not working anymore.
