It would have been more helpful if you had shared the settings you used, like I did.
My guess is that you used "save new with other unique name"

However, I suppose after a reboot it's no longer "enabled"? Hence the need to use /RunFldMon as you described? Hi Fletch,

Skip pages option means that OCR operation will not be applied to page that contains text objects, other pages will be processed.
Second option means that if any page contains text, then OCR operation will not be applied to whole PDF document.

you can customize target image filename by using %[FileName]_%[Page] macros to save your file. %[FileName] is source PDF name (in your case it is Top level bookmark name) and %Page is number of page in this PDF document.

you can apply Split Pages tool to your PDF documents. Just add vertical split in Split Pages action's options
Or you can use custom tool from attachments.
This custom tool Creates PDFs from scanned images and then split its pages vertically.
