I have been using this product for many years and didn't realize that when you have Thin Lines turned on, that when you print out a document, it will only print those thin lines, as opposed to the original line weights. I am running build 313.1 - June 8 2015 and still have the same or similar issue as described in the original post.
I can get around this by showing the scanners native interface when scanning, but that is an extra step that I would like to avoid.

Perfect, just was I was looking for. I had tried dragging the tools from the customize toolbar dialog box to the new tool bar I had made, but I didn't realize you had to drop them right on the very left edge of the new tool bar. Once I got the first tool dragged, it was easy to drag the rest. Thanks
